For timely enrollment, please return all completed forms along with the student’s birth certificate, shot record, proof of residency, last report card and schedule, if available, to Bradford Area School District's Enrollment Secretary, District Administrative Office, 150 Lorana Avenue, Bradford.

1.  Student entry form. (complete and return)
2.  Release for records/transcript of grades. (complete and return)
3.  Student information. (complete and return)
4.  Health history/physical exam. (complete and return)
5.  Parental registration. statement (complete and return)
6.  Home language survey. (complete and return)
7.  Proof of residency. (verification documentation)
8.  Proof of immunization. (verification documentation)
9.  Proof of age. (verification documentation)
10. Parent/guardian photo identification.

Per State of Pennsylvania regulations, the following documentation is required before a student can be enrolled:

    • Proof of child’s age
    • Immunization record
    • Proof of residency
    • Parental registration statement
